Pre-Screening Questionnaire
This is our MHSG Pre-Screening Questionnaire and it is a tool we use to determine whether or not a potential client should fill out out New Client Packet or NCP.

New Client Packet
The NCP is the greatest tool we have here at MHSG. It is important that it is filled out correctly and verified for completion before being submitted to us. It takes what is normally a 150+ question intake on the Social Security Website and pairs it down to about 36 questions. Housed Client Page
MHSG specializes in clients who are experiencing homelessness, but we are always happy to help anyone experiencing a long-term, permanent disability file for Social Security. In the event that a client who is currently housed would like to file with us, they will need to complete this page which details their current resources, in addition to the New Client Packet (NCP)
